As part of the integrated Health & Social Care intermediate Care Service we have developed Positive Step, an Adult Social Care Assessment unit in Bentley, which will be operating with approximately 33 beds. 

The service takes referrals from an Integrated Discharge Team at Doncaster Royal Infirmary. The unit offers short term accommodation for people who require a comprehensive assessment and works towards building confidence and skills to enable people to return home. This work is also supported by Social Care and Therapy staff. 

The service is looking to appoint two 30 hour permanent care coordinator posts who will support the Management team in the shaping and design of the service. 

This is an exciting opportunity for an individual who has had experience dealing with completing care assessments and applications in line with Mental Capacity Act. This is a challenging role working with a diverse and fluid client group along with interacting with a number of professionals at all levels. The environment can often be fast paced with daily challenges posed.

It will require the ability to be able to work and act quickly and think creatively to improve, maintain and manage the admissions an ever pressurised service. It is essential that Innovation and ability to embrace change is demonstrated and that a clear understanding of multi-disciplinary working. Medication compliance and competency is to your advantage. 

This post will therefore need someone with previous experience in a care environment ideally working within a multi agency approach. It will require the ability to be able to work and act quickly and think creatively to improve, maintain and enhance a transient service. 

They will need to lead by example and ensure that all staff are competent in being able to deliver rehabilitative care in line with service and Care Quality Commission standards. 

As I am sure you will all agree that this is a fantastic opportunity as the growth in Intermediate Care services is huge and the introduction of the permanent adult social care assessment unit will offer us the opportunity to demonstrate how we can adapt to the changing needs of our service users and commissioners and embrace the opportunity to work collectively with joint services.
